This section is used to indicate which type of contract template you are requesting.
This is the most common kind of public works contract template. It is used for most small construction and maintenance projects.
Selecting this option will populate a BID/RFP/RFQ procurement number field. Please enter the procurement number in this field. This number will be in, “XX-FYFY,” or “CAXX-FYFY,” format, with, “XX,” representing the unique number and, “FYFY,” representing the fiscal year of the award. The contract template will be part of the BID/RFP/RFQ documents.
Certain special services have contract templates unique to that field. The most common of these include architectural services agreements, engineering services agreements, inspector of record, laboratory of record, construction management agreements, and agreements for geotechnical engineering. Selecting this contract type will populate radio buttons for each of these types of agreements. If the special services you are purchasing are not on this list, then select, “Independent Contractor Agreement.”
This contract template is used for most non-construction contracts. If you are unsure what to select, select this contract type.
Selecting this contract type will populate a field to enter the existing PO number and a radio button to remind you to upload the original contract to the requisition as backup along with the PO change form and vendor proposal for the change order. We may have a copy of the original contract in our purchasing files, so if you do not have it, please provide as much information as you can and we will try to locate it.
Selecting this contract type will populate a date and text field. In the date field, enter the date that the form of the vendor’s contract was sent to General Services for review. The text field is there so you can provide an explanation as to why the district standard boilerplate contracts were not sufficient for use. This is typical for some contracts such as software licensing agreements.
