MANDATORY COVID-19 COMPLIANCE DECLARATION

All employees are required to declare their vaccination status by submitting a COVID-19 Compliance Declaration form.  All employees have been e-mailed a notice of this requirement and a link to the form.  If you still need to complete the declaration, you may also use the link below:

EMPLOYEE VACCINE MANDATE EXEMPTION REQUESTS

Religious Exemptions - Employees that object to the District's vaccine mandate based on a sincerely held religious belief must read and complete the Religious Accommodations Request Form for COVID-19 Vaccination below.  Once complete, please submit the form to Samantha Folb, Director of Training, Compliance, & Employee relations at samantha.folb@wvm.edu.

Medical Exemptions - Employees that would like to request a medical accommodation to the District's vaccine mandate must contact Quyen Lenshoek, Human Resources Specialist, at lequen.lenshoek@wvm.edu
