Report a Concern
These reports are designed to provide members of the West Valley - Mission community with an online method to report community concerns and get connected to resources.
Care Report
Report a concern about a student’s well-being. This form can be used to connect students to resources like health and wellness services, financial services, academic resources, DSPS, and EOPS.
Incident Report
Report an incident/concern including disruptive behavior, student grievances, accessibility/access
issues, general campus concerns, or COVID-19 concerns.
Title IX (Sexual Misconduct) Reporting Form
Report sex-based harassment or discrimination including sexual harassment, sexual violence, intimate partner violence, and stalking. Check out our Title IX for more information.
Unlawful Harassment/ Discrimination/ Retaliation Reporting Form
Report alleged incidents of unlawful harassment, discrimination, or retaliation based on someone’s affiliation with a protected class (national origin, religion, age, gender, gender identity, gender expression, race or ethnicity, color, medical condition, genetic information, ancestry, sexual orientation, marital status, physical or mental disability, veteran or military status, or pregnancy).
Academic Integrity
Report an issue related to academic integrity. This may include cheating, plagiarism, and/or inappropriate use of artificial intelligence (AI).
Complaint via the CCC Chancellor's Office
You also have the option to file a complaint through the California Community College Chancellor's Office (CCCC) legal division.
